Behavioural History & Risk Disclosure

Mandatory disclosure: You must disclose every known bite, attack, threatening behaviour, escape attempt, council investigation, declared-dog status and trigger. Withholding material information may result in refusal of admission, suspension of training, immediate collection requirements and liability for reasonably incurred costs.

Legislative Compliance Declarations

The statements below support admission screening and do not replace the owner’s legal duties under the Domestic Animals Act 1994, local laws, registration conditions or declared-dog requirements. Notify The GSD K9 Training immediately if any answers change before or during the program.

I declare that the dog is currently registered with the council named in Section 3, unless a lawful registration exemption applies. Evidence may be requested before admission.

I declare that the microchip number supplied is correct and identifies the dog presented for admission.

I declare that the dog is not a restricted-breed dog and is not subject to a dangerous dog declaration, menacing dog declaration, guard-dog classification, seizure order, destruction order or other restriction, unless disclosed below in full.

I understand a dog may be refused, isolated, have training modified, or require collection if it is ill, unsafe, unsuitable for the program, or if supplied information/documents are incomplete or inaccurate.

I authorise The GSD K9 Training to contact my veterinarian, relevant council or an emergency contact to verify information, respond to an emergency or protect the welfare and safety of people and animals.

Emergency Veterinary Treatment Authority

I authorise The GSD K9 Training to provide first aid within staff competence and to seek urgent veterinary examination, treatment, diagnostics, hospitalisation, anaesthesia, surgery or other care reasonably considered necessary for my dog’s health, welfare or safety when timely contact with me is not possible.

I authorise transport of my dog to my usual veterinarian, the establishment’s on-call veterinarian, or the nearest appropriate emergency veterinary service.

I accept responsibility for all veterinary, medication, transport, emergency-care and reasonable related costs incurred for my dog, except to the extent that loss or expense is caused by the business’s negligence or a right cannot lawfully be excluded.

Owner Consent, Acknowledgment & Liability Terms

Assessment and suitability: I understand Board & Train is not a guarantee of a particular behavioural outcome. Training results depend on the dog’s health, genetics, history, environment, consistency, owner implementation and the quality of follow-up handling. The trainer may pause, modify or discontinue training where welfare, safety or suitability requires.

Inherent risks: I understand that working with dogs involves foreseeable risks, including stress, illness, injury, escape attempts, interactions with people/animals, transport and environmental hazards. I have disclosed all material health and behavioural information so risks can be assessed and managed.

Release and indemnity: To the maximum extent permitted by law, I release The GSD K9 Training and its personnel from claims for loss, damage, injury or expense arising from the ordinary and inherent risks of the program, except where caused or contributed to by the negligence, wilful misconduct or breach of a non-excludable legal duty of the business or its personnel. I agree to indemnify the business against third-party claims or costs arising from my dog’s undisclosed history, inaccurate information or breach of owner's legal obligations, to the extent permitted by law.

Owner participation and handover: I understand I must attend or complete the agreed handover, coaching and follow-up instructions. I accept that the dog’s ongoing progress and safety after discharge depend on consistent owner implementation and appropriate management.

Privacy and records: I consent to The GSD K9 Training collecting, using and storing the information in this form to provide services, meet welfare and legal obligations, contact veterinarians/emergency contacts and maintain business records. Information will be handled in accordance with the business privacy policy and disclosed where authorised, required by law or reasonably necessary for safety and welfare.

Owner Declaration & Signature

By signing, I confirm that I am the legal owner or authorised person responsible for the dog, that I have read and understood this form, and that the information supplied is complete and accurate to the best of my knowledge. I agree to notify The GSD K9 Training immediately of any change to my dog’s health, vaccination status, legal status, behaviour or contact information before or during the program.
